TAG, TISSUE SPECIMEN
The General Services Administration has authorized the use of this commercial item description.
This commercial item description covers a data tag for use with tissue specimen bags.
Salient characteristics.
Shall be a tear-proof, water-proof and ink smear-proof data tag for use with tissue specimen bags. Shall include a pressure sensitive tape on the backside for attachment to a tissue specimen bag.
Shall be perforated to allow removal of a file copy containing an applied identification number with clinical information while leaving a taped stub portion with an applied matching specimen number on a tissue specimen bag. The perforation shall divide the data tag into 3 x 5 inch side data filling card with a detachable stub. The backside of the stub shall have a pressure sensitive tape for attachment to a tissue specimen bag. The pressure sensitive tape shall be supplied with a removable protective liner.
Shall include provisions and appropriate legends for a designated specimen number to be marked on both the stub and file card portions of the data tag. The file card shall include provisions and appropriate legends for the following type of data to be marked on the file card:
Patient name
Surgeon name
Specimen/culture site
Diagnosis
Clinical history
Tests requested
